After nearly one and a half years, our hospital witnessed the delivery of a critically ill preterm baby, marking a moment that tested both preparedness and precision. Born with a weight of just 2.120 kg, this newborn entered the world under extreme urgency. There was no time to administer antenatal dexamethasone to the mother or wait the recommended 48 hours for fetal lung maturation—making the situation even more challenging from the very start.
The mother herself was in a severely compromised state, battling congestive cardiac failure with an alarming lactate level of 20, indicating profound physiological distress. The delivery became a race against time, with both maternal and neonatal lives hanging in delicate balance.
At birth, the baby showed no signs of life—no heart rate, no spontaneous breathing, and completely limp. The newborn was likely already compromised in utero due to the mother’s critical cardiac condition. In that crucial window, every second mattered. Without delay, the neonatal team, led by Dr. Snehal Mehta, HOD – Pediatric & Neonatal Intensivist, Marengo CIMS Hospital, Ahmedabad, initiated immediate and advanced resuscitation.
For nearly three intense minutes, cardiopulmonary resuscitation (CPR) was administered—moments filled with urgency and determination. Then, against the odds, a heartbeat returned. Breath followed. Life, once on the brink of loss, was restored.
What makes this case truly extraordinary is not just the successful resuscitation, but the outcome that followed. The baby survived with intact neurological function—a rare and remarkable recovery in such high-risk circumstances. Over the next few days, the newborn showed steady improvement. The baby was gradually stabilized, successfully extubated, weaned off oxygen support, and began tolerating feeds well. From a lifeless beginning to active responsiveness, the recovery was nothing short of remarkable.
Yet, beyond the clinical success lay a deeply human challenge. With the mother requiring intensive care and the baby admitted to the NICU, the financial burden on the family became overwhelming. Despite the positive progress, the family made the difficult decision to opt for discharge against medical advice.
At the time of discharge, the baby was stable, active, off respiratory support, and feeding well—a testament to the timely intervention, clinical excellence, and compassionate care delivered by Dr. Snehal Mehta and the entire neonatal team.
This case stands as a powerful reminder that even in the most critical beginnings, life can prevail—when expertise, swift decision-making, and unwavering commitment come together.
